SECTION .0400 – REPORTING AND DISCLOSURE REQUIREMENTS

21 NCAC 25 .0401             DUTY TO REPORT CHANGES IN PERSONAL INFORMATION

A licensee shall notify the Board in writing of any change in the licensee's current:

(1)           Name;

(2)           Home street address;

(3)           Home mailing address;

(4)           Home and work telephone numbers;

(5)           Home and work fax numbers; and

(6)           Email address;

within 30 days after the change occurs.
